A Vial of Life Kit contains important medical information about you that may be needed by those responding to an emergency.  It's necessary to keep your information current.  If you have a Vial of Life and need to update your information you may obtain a blank Vial of Life Form by clicking here.

PATIENT SATISFACTION SURVEYS- NOW ONLINE

For over five years now, Kirby Medical Center has been sending patient satisfaction surveys to randomly selected patients. Those surveys have been administered on paper through the mail and filled out by hand.

However, in addition to the paper surveys, Kirby Medical Center began administering patient satisfaction surveys electronically via email in August. Randomly selected patients will receive a survey in either paper format or email format, but not both. Kirby Medical Center hopes that the availability of the survey in email form will add an additional convenience to patients as part of the hospital’s continued efforts to make it easier for them to submit feedback about patient experiences.

Both surveys contain the exact questions - regardless of the survey delivery method. And, as with the paper ones, the e-surveys are completely confidential.

FREE BLOOD PRESSURE SCREENING

The Kirby Auxiliary hosts a FREE Blood Pressure screening every Tuesday in the Promenade at Kirby Medical Center from 9am until 11am.  

It's fast, easy and the best part...it is FREE.  While you are here, why not stop by the Gift Shop where you'll always find new and exciting things in store. 

Please Note:  Blood Pressure Screenings will not be held on December 26, 2017.  Thank you!

If you are in need of transportation to Kirby Medical Center for blood pressure screenings (or to donate blood), you may want to consider calling Piatt County Public Transportation (Piattran) at 762-7821.  Registered Seniors may be able to ride for free.  You may call Piattran Monday - Friday from 6AM to 6PM for more details.

KIRBY AUXILIARY TO HOLD YOGA CLASSES

Beginning on February 21, 2017, the Kirby Auxiliary will again hold Yoga Classes on Tuesday evenings at 6:30pm in the Kirby Medical Center classrooms.  Cost for each class is $10.  Please contact Norma Wesley via email allikatwine@gmail.com or phone 217-762-8041, if you plan to attend.  Thank you!

FREE KIRBY DERBY TRAINING PROGRAM

Training Program Kick-Off begins March 14th at 5:30 pm at Kirby Medical Center Wellness Trail. 

If you have a desire to walk a 5K, to start running, become a better runner, or if you are interested in meeting fellow runners to help motivate and support your ongoing training. Join our 8-week training program designed for every from beginning walkers to experienced runners. 

Program Details

  • Every Tuesday & Thursday at 5:30pm from March 14th-May 4th training walks/ runs at Kirby Medical Center Wellness Trail
  • FREE and open to the community
  • All ages 8 years and older welcome to participate in 5K/10K training.

2017 KIRBY DERBY

The Kirby Derby is a family friendly 5K Run/Walk and a 10K Run, along with a FREE Kids’ Race. The race takes place in the Forest Preserve Park on Saturday May 6th, 2017 at 8:00AM. Participating in the Kirby Derby is a great way to maintain a healthy and active lifestyle while having fun. All the proceeds go to the development of the Wellness Trail provided by the Kirby Foundation. For more information and online registration click on the links above.

The BIG NEWS for 2017 is the addition of the Mini distance triathlon.

Sage City Mini Triathlon

Consists of a 100 meter outdoor pool swim, a 5 mile bike and a 1 mile run on the shaded path.

This is the perfect distance for youths and for  anyone wanting to Try a Tri. The On Line registration link is below.

The Kirby Medical Center/KMC Sage City Triathlon will include a 300 meter outdoor pool swim, 13.5 mile bike ride, and a 3.1 mile run. This USAT certified course will start in the new Monticello city pool using a serpentine swim route. The bike route will highlight one of Monticello’s prime attractions; Allerton Park. Riders will enter the park along the recently completed South entrance. The route circles the famous Sunsinger statue at the West edge of the park, and offers beautiful views of many park features. The run is a flat, fast course along a closed jogging path that is heavily shaded and travels over Monticello’s new wooden bridge spanning the Sangamon River. The run is paved for approximately two miles and is hard packed gravel for one mile. The surface variation offers terrific relief for tired feet. Expect PR run times.
