Get Game-Ready: Kirby Medical Center Offers Sport Physicals This Summer
As student athletes across Piatt County prepare for another exciting season, Kirby Medical Center is proud to support local youth with accessible and affordable sport physicals at convenient locations throughout the community.
From May through August 2025, families can take advantage of scheduled sport physical events hosted at area schools and Kirby Medical Group clinics — all for just $25 per student.
But that’s not all: $20 from each physical will be donated back to the school’s booster organization, helping support your student’s team while ensuring their health and safety.
2025 Sport Physical Schedule
Kirby Medical Center is partnering with local schools to provide on-site sport physicals at the following times and locations:

What You Need to Know
Cost: $25 per student (cash or check only)
Boosters Benefit: $20 from each physical is returned to the student’s school booster organization
What to Bring:
A completed sports physical form signed by a parent or guardian
Payment (cash or check only)
These physicals meet Illinois High School Association (IHSA) and Illinois Elementary School Association (IESA) requirements and are valid for 13 months.
Why Sport Physicals Matter
Sport physicals (also called pre-participation physical examinations) are a crucial step in keeping young athletes safe, helping identify any health concerns that might affect performance or risk of injury.
