Hit The Ground Running With A Sports Physicals

Where: Kirby Medical Group- Monticello
When: (by walk-in from 3-6pm)

• May 16th - 4:00 pm - 6:00 pm at Argenta-Oreana High School
• June 13th - 3:00 pm - 6:00 pm at Kirby Medical Group in Monticello
• July 22nd - 3:00 pm - 6:00 pm at Monticello High School
• August 7th - 3:00 pm - 6:00 pm at Monticello High School

Please remember to bring a sports physical form signed by a parent or guardian and $25 cash or check. $20 for each athlete is returned to their respective booster organization.
