Sport Physicals
Gearing up for sports season? Student athletes can complete their sport physicals at one of our local events in May through August.
Please bring $25 cash or a check. We’ll return $20 for each athlete to their respective booster organization. If you attend, please remember to bring a sports physical form signed by a parent or guardian.
