Kirby Medical Center’s Mobile Integrated Health (MIH) program is
designed to provide high-quality, in-home healthcare services delivered
by trained ambulance professionals. This free program is ideal for individuals
managing chronic conditions or living alone with minimal support.
Our Services Include:
- Vital Sign Monitoring: Blood pressure, heart rate, respiratory checks,
lung sounds, and blood glucose readings.
- Medication Review & Pickup: Ensuring medication accuracy and adherence.
- Post-Hospitalization Follow-Up: Helping patients transition safely back home.
- Care Coordination: Connecting you with healthcare providers, social workers,
and local resources.
- Emergency Response: Medical care and transport when urgently needed.
- Mental Health Support: Initial screenings and referrals to appropriate
mental health services.
Who is Eligible?
- Individuals with current medical conditions
- Those living alone with infrequent family contact
This program is free of charge and includes regular visits either bi-weekly
or monthly, depending on your needs.
